The authors take advantage of unique Danish birth and psychiatric registries to test the hypothesis that Rhesus incompatibility is a risk factor for schizophrenia. The rate of schizophrenia is increased in men who are Rhesus incompatible with their mothers and in second and later born incompatible offspring.

A famine of brief and clearly defined duration in a specific geographic area near the end of the Second World War allowed investigators to explore prenatal nutritional deficiency as a risk factor for schizophrenia. The most exposed cohort conceived at the height of the famine showed a two-fold increase in risk.

Am J Psychiatry 1995, 152:699-703. The Hillside First Episode study includes rigorous clinical and biological assessments of a well defined cohort treated in a standardized medication protocol. Careful attention to methods render the authors' finding that a smaller percentage of men than women achieve complete symptom remission compelling.

As follow-up studies from specific institutions are subject to sample selection bias, many basic questions about the natural history of schizophrenia have not been answered definitively. This study describes outcome among an epidemiologically based sample and fails to replicate the relationship between female sex, later age of onset, and better long-term outcome described by others.

This is a carefully conducted prospective study that convincingly shows an association between better treatment response and shorter duration of untreated psychosis for neuroleptic-naïve patients and greater consistency of neuroleptic treatment for chronic patients.

Given early and aggressive use of antipsychotic medications throughout most of the world, it may only be possible to study the impact of long duration of untreated psychosis on outcome in patients from the preneuroleptic era. The authors found a relationship between duration of untreated psychosis and severe negative symptoms late in the illness course of schizophrenia.

42. McGlashan TH, Johannessen JO: Early detection and intervention with schizophrenia: rationale. Schizophr Bull 1996, 22:201-222. In an issue of Schizophrenia Bulletin devoted to early detection and prevention of psychosis, the authors carefully review evidence supporting an association between delayed treatment and poor long-term outcome in schizophrenia. Despite the tentative nature of available data, a compelling case for further exploration of early detection strategies is outlined.

44. Falloon IRH, Kydd RR, Coverdale JH, Laidlaw TM: Early detection and intervention for initial episodes of schizophrenia. Schizophr Bull 1996, 22:271-282. The group describes what may be the first early detection and prevention program developed in a small catchment area in the UK. Although data suggesting a reduced incidence of florid schizophrenia after implementation of the program are highly tentative, the authors present innovative clinical and public health perspectives.
