The authors take advantage of unique Danish birth and psychiatric registries to test the hypothesis that Rhesus incompatibility is a risk factor for schizophrenia. The rate of schizophrenia is increased in men who are Rhesus incompatible with their mothers and in second and later born incompatible offspring.

A famine of brief and clearly defined duration in a specific geographic area near the end of the Second World War allowed investigators to explore prenatal nutritional deficiency as a risk factor for schizophrenia. The most exposed cohort conceived at the height of the famine showed a two-fold increase in risk.

Given early and aggressive use of antipsychotic medications throughout most of the world, it may only be possible to study the impact of long duration of untreated psychosis on outcome in patients from the preneuroleptic era. The authors found a relationship between duration of untreated psychosis and severe negative symptoms late in the illness course of schizophrenia.
