Rheumatic manifestations of occult cancer

Naschitz JE, Yeshurun D, Rosner I: Rheumatic manifestations of occult cancer. Cancer 1995, 75:2954-2958. In 108 patients admitted with unclassified rheumatic symptoms, malignancy was diagnosed in 25 patients (23.1%) during a 2-year follow-up period. Malignancy was discovered on routine examination in 19 patients and following a specific search in four patients. In two cases, Hodgkin's lymphoma was diagnosed 6 and 12 months, respectively, after initial presentation. The associated clinical features, spectrum of rheumatic symptoms, and response to therapy are discussed.

Whitmore SE, Watson R, Rosenshein NB, Provost TT: Dermatomyositis sine myosltis: association with malignancy. J Rheumatol 1996, 23:101-105. In this series the cases of seven patients with classic dermatomyositis and 12 with dermatomyositis sine myositis are analyzed. Maligancy was diagnosed in 25% of patients with dermatomyositis sine myositis and in 27% of patients with classic dermatomyositis. This study highlights the entity of dermatomyositis sine myositis and its association with malignancy.

The association of mixed cryoglobulinemia with HCV and HBV infection is well documented. This study is the first report of the development of hepatocellular carcinoma in three of 250 patients with mixed cryoglobulinemia. All three patients had evidence of hepatotropic virus infection and the hepatocellular carcinoma occurred after a long duration of mixed cryoglobulinemia.

Clin Exp Rheumatol 1995, 13(suppl 13):161-163. The authors report a prospective study of 41 patients with cirrhosis alone and 41 patients with cirrhosis and hepatocellular carcinoma. They show that patients with cirrhosis are at high risk for hepatocellular carcinoma, especially if they have associated HCV and HBV infection. They also show a strong association between HCV infection and cryoglobulinemia in patients with cirrhosis with and without hepatocellular carcinoma.

The authors report on 14 patients in a series of 200 patients with mixed cryglobulinemia who developed a B cell non-Hodgkin's lymphoma. Non-Hodgkin's lymphoma appeared 1 to 11 years after mixed cryoglobulinemia was diagnosed. All 14 patients had anti-HCV antibodies and 12 of them had evidence of ongoing HCV infection. The authors comment that their study supports the role of HCV in the pathogenesis of mixed cryoglobulinemia and propose that it may be important in the transition of mixed cryoglobulinemia from a benign to a malignant disorder.

Median nerve compression is a common nerve entrapment syndrome and is idiopathic in many patients. Two patients are described who presented with carpal tunnel syndrome as one of the initial manifestations of T cell lymphoma. The importance of histologic examination of the annular ligaments is emphasized.
