Using in situ hybridization, the authors studied the possibility that lymphoproliferation in SS arises within a subset of B cells and may have prognostic information. Fourteen patients were studied. Seven patients had light chains in the tissue. Of these seven patients, five developed lymphoma after the study onset. One was lost to follow-up, and one was alive without evidence of neoplasia.

Using the meta-analysis technique of all case-control and cohort studies to test whether myositis precedes or follows cancer diagnosis, the authors identified 1078 myositis cases (565 PM and 513 DM) and a comparable number of controls. Data from each study suggested an association of both PM and DM with cancer. Overall odds ratio for the association of cancer with DM was 4.4, and for PM, 2.1. Cancer risk was high both before and after DM diagnosis, but for PM, cancer risk was increased only after diagnosis of myositis.

Fifteen patients with rheumatoid arthritis and three with DM who developed lymphoid neoplasms were studied to evaluate how frequently lymphoid neoplasms in such patients show features of lymphoproliferation that occur in immunocompromised patients. Eleven patients were receiving methotrexate at the time they developed lymphoproliferative lesions. Of these patients, 73% showed histologic features common to those occurring in known immunodeficiency states. In addition, 33% of all lymphoid neoplasms in these patients had Epstein-Barr virus genome and protein product, a well known factor associated with lymphoma. The results of this study indicate that a subset of lymphoid neoplasms in such patients shows features associated with immunosuppression.

The authors describe three patients with non-Hodgkin's lymphoma who presented with polyarthritis, and they reviewed the literature and found seven additional cases. They discuss the diagnostic difficulties that may be encountered in such patients. Most lymphomas were of T cell origin. In the authors' three patients with non-Hodgkin's lymphoma and polyarthritis, there was no lag period; the authors suggest that arthritis and lymphoma were causally linked. Moreover, in the cases in which lymphoma was successfully treated, the arthritis symptoms subsided.
