Ann Intern Med 1996, 124:699-707. Open, randomized clinical trial of delayed versus immediate introduction of DMARD treatment. In an intention-to-treat analysis, patients on immediate treatment had statistically significant and clinically relevant better scores with respect to physical functional disability, pain, joint score, and erythrocyte sedimentation rate and occurrence and progression of radiologie changes at 12 months. While only 8% of patients on immediate DMARD treatment stopped treatment usually because of adverse reactions, 29% of patients in the delayed DMARD group stopped the non-DMARD regimen due to ineffectiveness.

These authors confirmed the known association of men with fewer than six tender joints at baseline in remission. It was not possible however to construct a prediction rule of enough certainty to be useful in clinical decision making. The rate of remission in patients with inflammatory polyarthritis after 2 years was only 25% and only 9% of all patients were in "sustained remission" after both 1 and 2 years.

Br J Rheumatol 1996, 35:1101-5. Although 25% of patients fulfilled at least once the criteria for remission during the study period, only 15% fulfilled the criteria at two consecutive visits. These exemplary case studies show that visits in which patients are in remission are not always consecutive. Remission thus may be best understood as a temporary state in the continuum of systemic inflammation or disease activity.

These authors clearly showed that what physicians describe as disease activity and criteria that they employ in their decision making with regard to DMARD-treatment can be measured with a few weighted variables. With an algorithm termed Disease Activity Score (DAS), which integrates the number of swollen, tender joints and sedimentation rate, disease activity can be quantified on a continuum from zero to ten. Depending on initial disease activity, a change of 0.6 to 1.2 represents a clinically relevant change.
