There is also an extensive discussion of possible mechanisms for how the various mutations in rhodopsin bring about different symptoms predicted by changes in its secondary and tertiary structure.

The paper is interesting because of the history of the Nougaret family, the pedigree, and the identification of the possible index case in Jean Nougaret, a butcher who lived in southern France from 1637 to 1719.

This is an example of the selection of a candidate gene for study based solely on the specific physiological function of the protein that it encodes. The symptom of Oguchi disease is the prolonged delay of recovery of the rods from light exposure. The arrestin and rhodopsin kinase genes are both involved in speeding up the recovery of bleached rhodopsin. Therefore, they were both prime candidates for mutation in Oguchi disease. Mutations in rhodopsin kinase associated with Oguchi disease are described. Mutations in the arrestin gene were described in a previous paper [38].

Previous linkage analyses localized the gene responsible for Leber's congenital amaurosis (LCA) to a 1 cM region of chromosome 17p13, and positional cloning work provided a set of 12 YAC clones in this region. There happened to be five candidate genes that had previously been mapped to chromosome 17p13 and each of these was tested by hybridization for its presence in the set of YACs. Four of the genes were not present, but the fifth, retGC (retinal guanylate cyclase), was. The exon/intron boundaries of the gene were then determined on the basis of the work done on the mouse gene, and the exons were screened by SSCPE for mutations that were found in at least four families. In addition, the possible effect of retGC mutations in the pathology of LCA is proposed, and the point is made that there is at least one other LCA locus and one other guanylate cyclase gene.

This report provides a comprehensive history of the discovery of the mutant Rds murine gene, and a description of the protein it encodes. The mutation spectrum is presented along with an attempt to correlate phenotype with the position of the mutation in the protein in the extremely heterogeneous group of dystrophies caused by peripherin/RDS mutations. This task is helped by an excellent organization of peripherin/RDS phenotypes into four broad categories: RP, severe macular degenerations, pattern-type dystrophies and digenic RP (the first three categories are autosomal dominant). However, for this particular gene, the phenotype/genotype correlations are still quite complex and not yet clearly delineated.

An Arg142→Trp mutation was found only in affected members of all 7 families. However, no mutations were found in the exons of the 4 simplex cases. This paper is interesting for several reasons: first, the selection of the peripherin/RDS candidate gene based on its association with other macular dystrophies; second, yet another phenotype for peripherin/RDS mutations; third, the suggestion of a founder, as all 7, apparently unrelated, families carried the same mutation; finally, the implication of peripherin/RDS as one possible candidate for age-related macular degeneration.

The issue of why the heterozygous Leu185→Pro mutation in peripherin/RDS combined with a heterozygous null mutation in ROM1 causes photoreceptor degeneration while alone neither causes disease is tackled in this paper. The sedimentation velocity evidence presented suggests that when peripherin/RDS is normal, it can form a native-like homo-oligomeric membrane complex, even if a mutant ROM1 is present. However, in the case of the mutation, peripherin/RDS can form a functional hetero-oligomeric complex with normal ROM1, but not with itself nor with mutant ROM1. Extrapolating to in vivo digenic retinal disease, the mutant peripherin/RDS is dependent upon normal ROM1 to produce a functional disc membrane complex, but when the heterozygous ROM1 null is also present, an insufficient amount of normal protein is available to maintain disc integrity.

This paper presents the interesting story of how the study of a group of genes (21 in all), the ATP-binding transporters, led to the identification of a candidate for Stargardt's disease based on its chromosomal locus. A partial screening of the exons of the gene in a group of patients revealed mutations that were associated with disease in several families. In addition, in situ hybridization showed that the message corresponding to the gene is expressed in rods but not cones. This provides a bit of a mystery as Stargardt's is a disease that occurs primarily in the macula. Hypotheses are developed to try to explain why a rod-specific gene does not cause more disease in the periphery of the retina where the relative proportion of rods to cones is much higher than in the macula. The work also poses the possibility that other retinal dystrophies may be caused by other members of this family of ATP-binding transporter genes.

The discovery of the same TIMP3 mutation (Ser181→Cys) being responsible for Sorsby's fundus dystrophy (SFD) in 15 apparently unrelated families from different parts of the British Isles led these authors to consider a common founder for this mutation. Haplotype analysis revealed an allele of the marker D22S280 to be present in all affected individuals, while its frequency in control populations was only 11%. Further studies defined a common 3cM locus in 68% of all chromosomes studied. The frequency moved to 95% if the locus was expanded to 7 cM; perhaps this 7 cM region was the haplotype of the founder. This work suggests a common founder for at least the majority of SFD in the British Isles.

This includes the refinement of the locus based on various deletions, one of which led the work off in the wrong direction for some time. It also includes the identification of a good candidate gene in the locus, ETX1. This gene did not have mutations associated with RP3 XLRP, however. There is a description of this novel RPGR gene and extensive discussion of its possible function and relationship with homologous genes. The mutations found in 7 families are presented, and the authors also speculate about the fact that mutations were not detected in 67 additional families studied. They suggest that mutations were simply missed due to technical limitations, that mutations exist in regions not tested (such as introns or control regions), and/or there is another gene at the locus.

The authors provide a comprehensive history of the mapping of the locus, the positional cloning of the Rab escort protein 1 gene (REP1) and the discovery of its function. The mutations that have so far been found in this gene are catalogued; about one-quarter of European patients with choroideremia have large deletions, but these have not been found in North American patients (at least not yet). The remaining 20 mutations are either null mutations or splice mutations or small deletions or insertions causing frameshifts resulting in premature terminations. The absence of missense mutations associated with this disease suggests that only when the gene product is absent does choroideremia occur. The authors speculate that missense REP1 mutations may cause another form of retinal disease. In Europe, mutations in REP1 have now been identified in two-thirds of all patients with choroideremia.
