The use of the database from a large nationwide study gives two major advantages. First, selection bias for cases of strabismus is eliminated, and second, previously identified risk factors for comitant strabismus, such as increasing age of the mother, maternal cigarette smoking, low birth weight, and white race, can be taken into account to separately assess familial aggregation of comitant strabismus in the presence or absence of such risk factors. The study confirms that a statistically significant familial aggregation of comitant strabismus exists, even when the strabismus risk factors, which can themselves aggregate in families, are accounted for.

Patients identified with spasmus nutans, which is commonly considered a benign self-limiting condition that spontaneously resolves by 3 years of age, need to be carefully investigated and followed up, as they are at risk for developing visual impairment.

Clark RA, Miller JM, Demer L: Location and stability of rectus muscle pulleys. Invest Ophthalmol Vis Sci 1997, 38:227-240. The authors studied with multiple contiguous coronal HRMRI scans of 10 orbits of six normal subjects and 12 orbits of seven strabismic patients. Location of rectus muscle pulleys was inferred from extraocular muscle paths and was uniform across normal subjects. Radial shifts of the pulleys were found in secondary gaze positions in both normals and in patients with comitant strabismus. Two patients presenting with evident alphabetical pattern strabismus exhibited significant torsional dislocation of the pulleys. These findings, together with other observations concerning craniosynostosis syndromes and heavy-eye syndrome, strongly support the hypothesis that heterotopic pulley position is a potential cause of some forms of strabismus.
