The authors analyzed asthma deaths in a geographic area of the United States that has a wide range of climates and economic conditions. The most striking features of their study are that asthma deaths did not decline overall and that there was a progressive increase in asthma deaths in middle-aged women.

The introduction of long-acting bronchodilators although addressing a limitation of this type of drugs, has added a new set of concerns about its safety and proper use.

This study confirms the findings of several other investigators of a reduced baseline forced expiratory volume in 1 second following regular β-agonist therapy. It also adds another trigger to the list of factors that show enhanced bronchoconstrictor responses when patients are treated with an inhaled β-bronchodilator.

20. Hunt LW, Swedlund HA, Gleich GJ: Effect of nebulized lidocaine on severe glucocorticoid-dependent asthma. Mayo Clin Proc 1996, 71:361-368. An intriguing report on the use of a new approach to steroid-dependent asthma. The investigators were able to show a significant reduction in the steroid dosing, and in some cases there was a complete elimination of the glucocorticoid.
21. Joseph KS, Blais L, Ernst P, Suissa S: Increased morbidity and mortality related to asthma among patients who use major tranquillizers. BMJ 1996, 312:79-83. This study provides new information that the psychologic state or some aspect of stress can increase the risk of asthma. However, the results do not indicate that the drugs themselves are at fault. Management of psychologic stress in asthma is extremely important.
22. Atherton HA, White PT, Hewett G, Howells K: Relationship of daytime asthma symptom frequency to morning peak expiratory flow. Eur Respir J 1996, 9:232-236. This study suggests that the use of peak flow metering be reexamined because of the poor relationship between symptoms and peak flow determinations. However, the poor predictability of morning symptoms requires the use of objective measures of pulmonary function.
