The authors analyzed asthma deaths in a geographic area of the United States that has a wide range of climates and economic conditions. The most striking features of their study are that asthma deaths did not decline overall and that there was a progressive increase in asthma deaths in middle-aged women.

This study confirms the findings of several other investigators of a reduced baseline forced expiratory volume in 1 second following regular β-agonist therapy. It also adds another trigger to the list of factors that show enhanced bronchoconstrictor responses when patients are treated with an inhaled β-bronchodilator.

This study provides new information that the psychologic state or some aspect of stress can increase the risk of asthma. However, the results do not indicate that the drugs themselves are at fault. Management of psychologic stress in asthma is extremely important.

This study suggests that the use of peak flow metering be reexamined because of the poor relationship between symptoms and peak flow determinations. However, the poor predictability of morning symptoms requires the use of objective measures of pulmonary function.
