Ernst E, Resch K-L, White AR: Complementary medicine. What physicians think of it: a meta-analysis. Arch Intern Med 1995, 155:2405-2408. These investigators from the Center for Complementary Health Sciences in Exeter, England, evaluated 12 surveys of physicians' attitudes to complementary medicine. Such alternatives may be useful for patients, but more studies are needed to evaluate effectiveness. Physicians may use alternatives as helpful placebos, and it is currently concluded that there may be an increasing lack of conviction about the specific value attributed to individual alternative practices.

This is a very comprehensive review, and the author concludes that acupuncture is useful in bronchospastic diseases. Although the author is prejudiced in favor of alternative medicine (he is the European Editor of the new J Alternat Complement Med), the review should be given careful attention. Further controlled studies may help delineate the true value of this popular modality.

Refractory exercise-induced asthma in athletes may need to be evaluated with sophisticated studies, including isocapnic hyperventilation of frigid air, maximum exercise challenge, methacholine challenge, spirometry and maximal expiratory flow-volume curves, and direct laryngoscopy. Such careful studies are essential for the diagnosis of psychogenic vocal cord dysfunction.

Although patients with asthma are not significantly benefited by this therapy, smokers who ingest fish oil may have a slower rate of progression of chronic obstructive pulmonary disease.

Increased levels of nitric oxide occur in asthma and may cause bronchodilation by inhibiting cholinergic bronchoconstriction. However, nitric oxide may amplify the asthmatic inflammatory response to a greater degree.

Am J Respir Crit Care Med 1996, 153:122-127. Azelastine is a new anti-inflammatory drug with antihistaminergic properties. This study of 193 patients showed that azelastine was able to reduce dosing of inhaled beclomethasone by 25% to 30%. Because the drug also may reduce the need for β-agonists and theophylline, it may prove useful in more severe cases of asthma.
