Ann Intern Med 1995, 122:833-838. This is a well-performed study of 102 patients (mean age 62 years) investigated for either resistant hypertension or unexplained azotemia. Doppler ultrasonography did not effectively discriminate between 60-79% and 80-99% renal artery stenoses. The negative predictive value for disease of the main renal arteries was 97% in a population with a 71% prevalence of stenosis.

The innovative application of electron-beam computed tomography to examine cortical and medullary renal blood flow in patients with essential hypertension and renal artery diseases is reported. The results suggest differences in medullary blood flow alterations beyond stenotic lesions from fibromuscular disease compared with atherosclerotic disease.

This relatively large study (135 patients, mean age 54 years) used color-coding of blood flow in the estimation of renal artery stenosis and as a technique to identify renal vessels rapidly, reducing the examination time to 10-30 min. The negative predictive value was 98% in a population with a 21% prevalence of renal artery stenosis.

A comparison of MRA and renal artery Doppler ultrasonography in 55 patients (110 arteries) is given. The accuracy and technical failure rates were similar between methods. Both methods commonly missed accessory arteries.

This is an interesting decision analysis and estimate of the cost-effectiveness of examining for reversible renal arterial disease as a primary cause of end-stage renal disease. The authors argue that the cost savings from avoiding dialysis in a small number of patients justifies the use of MRA before angiography.
