J Clin Invest 1995, 96:1802-1814. This important paper provides direct evidence that increased glucose metabolism is the crucial parameter for increased matrix accumulation in mesangial cells. Cultured rat mesangial cells overexpressing the glucose transporter GLUT1 demonstrate markedly increased glucose uptake and metabolism as well as significant stimulation of matrix synthesis, even when the cells are grown in a normal extracellular glucose concentration.

This subgroup analysis of the DCCT study discusses the beneficial effects of intensive therapy on the development and progression of diabetic nephropathy in IDDM patients. Intensive therapy in patients with initial normoalbuminuria was associated with a decreased incidence of microalbuminuria. However, if microalbuminuria was present at entry into the study, intensive insulin therapy did not provide a statistically different effect compared with conventional therapy on the subsequent progression to macroalbuminuria.
