Assessment of coronary artery stenosis by magnetic resonance imaging

Pennell DJ, Bogren HG, Keegan J, Firmin DN, Underwood SR: Assessment of coronary artery stenosis by magnetic resonance imaging. Heart 1996, 75:127-133. This is the first detailed comparison of MRI and x-ray angiography in the assessment of characteristics of coronary artery stenosis. MRI was performed in 39 patients (55 coronary artery stenoses) with known coronary artery disease, and 85% of the stenoses were identified. There was very good agreement between the techniques for the distance of stenosis from arterial origin, indicating that the tomographic and projection techniques can be reliably compared and that signal loss by MR can definitely be ascribed to the location of coronary stenosis. A significant correlation was found between proportional MR signal loss at the stenosis and the percentage diameter stenosis severity, suggesting that this parameter can be used as an approximate guide to luminal narrowing. Stenosis length was overestimated by MRI, probably because of turbulence-related signal loss downstream from the stenosis, and the overestimation worsened as stenosis severity increased.

Patients were examined at rest and during adenosine infusion with MRI and Doppler velocity-flow measurements at cardiac catheterization. The limits of agreement between the two techniques were -13±30 mL/min (range, 18 to 161 mL/min) for measurement of flow and 0.1 ±0.4 mL/min (range, 0.7 to 3.7 mL/min) for flow reserve.

MRI was more precise than echocardiography (11 vs 26 g, P < 0.01) and more reproducible (8 vs 49 g, P < 0.01). The authors conclude that studies requiring demonstration of a change in left ventricular mass should be performed using MRI, because the higher per-patient scan cost is more than offset by the reduction in patient numbers required to achieve statistically significant differences (in order to detect a 20-g change in left ventricular mass with α = 0.05 and β = 0.10, 126 patients are required for echocardiography and five patients for MRI).
