This study describes the survival in an inception cohort of 408 patients with SLE followed up at Duke University. Overall survival was batter for white than black patients but was related to socioeconomic status, which was poorer among blacks. Higher age and male gender were also associated with decreased survival.

Disease onset was more severe in those with childhood-onset disease, who also demonstrated more antibody production. More adult-onset patients died, but they had been followed up for a longer period.

Am J Nephrol 1995, 15:123-128. This study outlines the outcome of 64 patients with end-stage lupus nephritis who underwent transplantation at the Mayo clinic. This study shows good kidney and patient survival overall, albeit shorter than other patients with SLE. It also demonstrates that immunosuppression with cyclosporine was better than with azathioprine alone. It is a retrospective study and does not provide information that might be helpful in comparing these patients with other lupus cohorts.

This retrospective study of a transplantation database provided information on patients with lupus nephritis who underwent transplantation in comparison with nondiabetic patients with end-stage renal disease. The analysis revealed that patients with SLE fared worst in the cyclosporine era, with a higher rejection rate. Although there is information related to kidney disease, it is hard to compare this cohort with other lupus populations.

It records the types of renal disease the patients had and their treatments and outcome in terms of patient and kidney survival. Risk factors for end-stage renal disease included WHO class IV and V, black race, and nephrotic syndrome. The analysis is univariate, without consideration of multiple factors.

This neurocognitive dysfunction was not related to active disease and thus may reflect consequences of previous disease and its treatment.

Am Heart J 1995, 129:510-515. This study of echocardiography in patients with SLE identified a 14% prevalence of pulmonary hypertension, which increased to 43% after a 5-year follow-up. Although this is an important observation, the study lacks the detail on the patient population that is required to compare these patients with other cohorts or to deduce the relationship of pulmonary hypertension to previous clinical disease in these patients.

This is a study of 74 premenopausal women with SLE, all of whom had been treated with steroids. Dual energy x-ray absorptiometry scanning revealed decreased bone density in comparison with 50 healthy controls and reference values. Unfortunately, we are not given enough information about the patients' disease, eg, whether they have had arthritis and what their level of disability has been.

J Rheumatol 1996, in press. In a study comparing SF-20 and SF-36 in 150 patients, both instruments were easy to complete. Correlation of scores between SF-36 and SF-20 was good in general. There appeared to be some correlation between disease activity measured by the British Isles Lupus Activity Group instrument and domains of the SF-36.

The authors studied whether changes in laboratory tests were predictive for clinical flares in SLE and concluded that the laboratory tests were not predictive. However, they did note some statistically significant P values, which they discarded because of multiple testing and which may not be necessary because they were testing features that had been shown previously. Moreover, their definition of flare may be too strict.
