To evaluate the accuracy of elevated concentrations of endotoxin in bronchoalveolar lavage fluid for diagnosing nosocomial pneumonia, the authors studied 71 bronchoalveolar lavage specimens from 66 patients. They demonstrated in this preliminary study that a cutoff value of greater than 5 endotoxin unit/mL for the concentration of endotoxin in bronchoalveolar fluid had a sensitivity and specificity of 100% and 75%, respectively, for the diagnosis of gram-negative pneumonia.
