Am J Respir Crit Care Med 1998; 158:3-11. This article presents a study of respiratory mechanics, partitioned between lung and chest wall components, in patients with ARDS after direct (n = 12) or indirect (n = 9) pulmonary insult. Derived results are consistent with a higher degree of consolidation in ARDS of pulmonary origin, which appears to be less amenable to recruitment. This report holds interest as the first clinical study to examine these two potential subtypes, but there is a high degree of mathematical derivation, and confirmation is needed from radiographic and histological data.

A protective-ventilation strategy (high PEEP, low VT, pressure limitation, permissive hypercapnia) shows possible benefits over conventional ventilation (low PEEP, high VT, normocapnia) in established ARDS, with significant differences in ICU mortality at 28 days, rates of weaning, and clinical barotrauma, but not in survival to hospital discharge. An earlier study [26] explains methods and measurements in more detail.

Experimental work using intra-abdominal balloon distension in anesthetized pigs is presented. Subjects with distended abdomen show greater oxygenation response to prone repositioning. This study is interesting in the light of other work suggesting that reversal of preferential ventral ventilation and/or impaired dorsal ventilation may be a mechanism of therapeutic effect in prone repositioning.

This report represents some of the first significant data 10 show that prone positioning may have a persisting mechanical effect on thoracic compliance and, therefore, potentially a therapeutic as well as supportive role in ARDS ventilation. Difference in response between patients may be related to baseline chest wall mechanics, and oxygenation response to repositioning correlates with mechanical changes.

Crit Care Med 1998, 26:15-23. Multicenter study of 177 patients randomized to placebo and five dosage groups (1.25-80 ppm nitric oxide). Numbers within groups are small (n = 8-34 in treatment groups). Short-term oxygenation changes are seen. Comparison of pooled data from treatment groups vs placebo provides no significant evidence of outcome benefit. Although the largest study so far, this is not a phase III trial and so the design does not allow a definite negative conclusion to be drawn.

    • 2 and so minimize intensity of ventilation in patients on inhaled nitric oxide; positive findings would have implied a mechanism of therapeutic benefit by reducing risk of ventilator-induced lung injury. In fact differences between oxygenation between groups were abolished within 72 hrs, possibly due to protective-ventilation strategy applied to both groups.

This blinded, randomized, controlled trial (n = 24) shows marked differences in ICU and hospital outcome between B patients receiving placebo and 16 patients treated with methylprednisolone starting at 2 mg/kg daily at an early stage (after at least 7 days of mechanical ventilation), continuing up to 32 days. Outcome appeared to be significantly improved in the treatment group, with no significant increase in infection.
