This review updates the molecular variants generated by POMC gene expression in the ectopic ACTH syndromes: the various forms of POMC mRNAs and the altered products of POMC processing. It also reviews our current knowledge of transcription factors acting on the rat POMC gene in AtT-20 cells. It shows the first data obtained by transfection of the human POMC gene promoter in the POMC-producing human SCCL cell line DMS-79.

The V1b (V3) receptor mRNA can be detected in many brain areas including the hypothalamus, and in some peripheral organs such as the heart, lung, adrenals, and kidney. Yet all these tissues have extremely low V3 receptor signals compared with the pituitary.

J Clin Invest 1993, 92:2790-2795. This study examines the in vivo and in vitro responses to various secretagogues of 10 patients with ectopic ACTH syndrome. It shows that the three bronchial carcinoids of the series responded in vitro to vasopressin with increased ACTH release. The same tumors contained functional glucocorticoid receptors as assessed by in vitro studies and Northern blot analysis. No pharmacologic study was performed to further characterize the subtype of vasopressin receptor. This paper provides interesting molecular clues to explain the pituitary-like behavior of ACTH-secreting bronchial carcinoids.
