Crit Care Med 1996, 24:334-337. This group, coordinated by Deborah J Cook in Hamilton, Ontario, Canada, recently started a series of publications in Critical Care Medicine. This paper is the first of 38 in this series. Three articles will be submitted shortly, focusing on the methodologic issues in assessing literature on prognosis in the ICUs, including one article on the use of outcome comparison to rate quality of care in those units.

This very interesting multicenter randomized study looks at the effect of information given to the doctors on the prognosis of the patients with the most severe conditions in the ICUs. The endpoint was treatment (quality of the information to the families, quality of death, respect of DNR orders, and so forth). The study shows no effect of the strategy.

This paper shows the importance of customizing the severity scoring systems in specific groups of patients, in this case patients with severe sepsis.
