This was a multi-institutional review of 53 fetuses found to have reversed atrial or ductal shunting by Doppler color flow mapping, in which follow-up revealed postnatal survival in only seven infants. The finding of reversed shunting in utero can provide a guide to accurate morphologic diagnosis and may have significant prognostic value.

Fetal sheep were administered acetylcholine, sodium nitroprusside, and endothelin-1 before and after cardiac bypass. There was significant diminution in the postbypass response to acetylcholine (endothelium-dependent vasodilator) and heightened response to endothelin-1. This is the first evidence that the placental vascular endothelium is an important factor in bypass-related placental dysfunction.

Fetal cardiac bypass using a miniaturized circuit with an in-line axial flow pump resulted in significantly improved placental blood flow, lower placental vascular resistance, and decreased acidosis and hypercarbia relative to a control circuit, with no pharmacologic inhibition of the arachidonic acid cascade or stress response. These results suggest that decreasing extracorporeal surface area and avoiding priming volume are effective means of improving postbypass placental function.

Eight of nine fetal sheep subjected to cardiac bypass without pharmacologic inhibition of the arachidonic acid cascade were delivered at full-term gestation. Autopsies performed after sacrifice at 1 week of age showed no gross hemorrhagic or thromboembolic lesions in the major organs and only minor microscopic pleural and hepatocellular changes in four lambs. Survival and delivery after fetal cardiac bypass is possible with only minor organ pathology.

This follow-up case report describes the prenatal and postnatal course of a child who survived after balloon dilatation of the aortic valve in utero for critical aortic stenosis. The patient was alive and well at 4 years of age, demonstrating that fetal intervention may become an important alternative in the management of complex congenital heart disease.

Fetal transesophageal echocardiography with intravascular ultrasound equipment was used to guide transumbilical catheter balloon dilatation of pulmonary stenosis simulated with by prior banding of the pulmonary trunk.
