This article evaluates internal MDO in infants with Robin sequence and severe obstruction, older micrognathic children with OSA, and tracheotomized patients with complex congenital syndromes. Success rates in each group were 88%, 82%, and 17% respectively. The authors recommend more conservative exclusion criteria for MDO in tracheotomized syndromic patients, and recommend more aggressive adjunctive airway procedures when applicable.

Tracheotomy avoidance was achieved in 14 of 15 selected newborns with micrognathia and airway obstruction who underwent bilateral MDO using internal microdistractors. Selection criteria, surgical technique, and postoperative management are well described.

This is the only published series on mandibular distraction for cerebral palsy-related airway obstruction. Of five carefully selected patients, three of four patients with tracheotomies were successfully decannulated and one child successfully avoided tracheotomy.

This clinical study demonstrated uniform success in 11 patients by molding the regenerate to correct occlusal relationships or to improve the anatomic position of distracted segments. Molding was accomplished through angulation of multidirectional distractors or the application of interdental elastics.

Oral Surg Oral Med Oral Pathol Oral Radiol Endod 2003, 96:392-397. This is a review of complications of internal MDO in 70 patients (mean, 14.2 years). Complications were seen in 28 patients (40%) and included condylar resorption, permanent sensory deficit, distraction failure from technical error, or device malfunction.
