This interesting preliminary study conducted in five patients demonstrated that a 6-week exercise program can double the diameter of the veins compared with control. This study needs to be repeated in a larger cohort.

A polyurethane vascular access graft and a hybrid polytetrafluoroethylene graft as an arteriovenous fistula for hemodialysis: comparison with an expanded polytetrafluoroethylene graft. Conclusions about possible differences between the graft materials are limited by the sample size.

This large randomized multicenter study compared use of vascular nonpenetrating clips with use of conventional sutures. Follow up at 2 years showed an absolute increase of 20% in patency for both AVF and AVG with use of vascular clips.
46. Miller CD, Robbin ML, Barker J, Allon M. Comparison of arteriovenous grafts in the thigh and upper extremities in hemodialysis patients. J Am Soc Nephrol 2003; 14:2942-2947. This is one of few prospective studies comparing access survival and complications in AVGs placed in the thigh versus those placed in the upper extremities. Technical failure was higher in the thigh, as was access loss due to infection. Cumulative survival did not differ.

Am J Kidney Dis 2003; 42:797-805. This study from Kuwait shows a higher than expected frequency of low protein C, protein S, and antithrombin III levels. In general, the lupus anticoagulant, protein C, protein S levels, and activated protein C resistance were significantly abnormal in those who experienced vascular access thrombosis episodes.

Randomized controlled trial of clopidogrel plus aspirin to prevent hemodialysis access graft thrombosis. J Am Soc Nephrol 2003; 14:2313-2321. A multicenter, double-blind, randomized controlled study that assessed the effect of antiplatelet agents on vascular access thrombosis is reported. The study was terminated early because of excessive bleeding in those receiving aspirin and clopidogrel.
53. Wali MA, Eid RA, Dewan M, Al-Homrany MA. Intimal changes in the cephalic vein of renal failure patients before arterio-venous fistula (AVF) construction. J Smooth Muscle Res 2003; 39:95-105. Vein tissue samples analyzed histologically at the time of AVF construction exhibited thickening of the wall and intimal hyperplasia in all patients. Less commonly inflammatory cell infiltration and mural calcification were also observed.

Highly increased cell proliferation activity in the restenotic hemodialysis vascular access after percutaneous transluminal angioplasty: implication in prevention of restenosis. Am J Kidney Dis 2004; 43:74-84. Surgical tissue obtained at the time of revision exhibited a higher proliferative index in those AVFs that had undergone previous PTA as compared with those with primary stenosis. The report emphasizes the need to evaluate antiproliferative therapy at the time of PTA.

Vase Endovascular Surg 2003; 37:259-269. An experimental canine model was used to examine the effects of an inhibitor of von Willebrand factor-dependent binding of platelets to collagen. Topical application at the venous anastomosis decreased intimal hyperplasia by as much as 77% compared with controls. Clinical application needs to be determined.
63. Moran JE, Prosl F. Totally implantable subcutaneous devices for hemodialysis access. Contrib Nephrol 2004; 142:178-192. A general review of the relative advantages of a subcutaneous port-catheter system over conventional tunneled catheters is provided. The authors believe that port-catheter systems may be used to bridge maturation of an AVF in those requiring larger maturation periods or more than one attempt at fistula construction.

65. Lopot F, Nejedly B, Sulkova S, Blaha J. Comparison of different techniques of hemodialysis vascular access flow evaluation. Int J Artif Organs 2003; 26:1056-1063. This study compared reproducibility and comparability of six different methods for measuring access flow. Ultrasound dilution had the best results and was used as the comparator for the others.
66. Sirken GR, Shah C, Raja R. Slow-flow venous pressure for detection of arteriovenous graft malfunction. Kidney Int 2003; 63:1894-1898. Normalized venous pressure at a low blood pump flow of 50 ml/min was as predictive of thrombosis as traditional static pressures.

Am J Kidney Dis 2003; 42:539-545. This is one of the few studies comparing colored Doppler with ultrasound dilution in native AVF. Both techniques had similar predictive power for detecting stenosis.

As in other databases, this study from India shows that creation of AVF lowers the risk for infection, even in those in whom hemodialysis is used as a bridge to transplantation.

This single center experience in a large number of patients gives some guidelines as to when subtotal or partial graft excision can be used to preserve the access site. Systemic sepsis mandates total graft excision.
