Comparison of clinical prediction models for resistant bacteria in community-onset pneumonia
-
Self WH, Wunderink RG, Williams DJ, et al. Comparison of clinical prediction models for resistant bacteria in community-onset pneumonia. Acad Emerg Med 2015;22:730-740. Recent head-to-head validation and comparison of multiple scores in a prospectively identified pneumonia cohort. Results are reported in terms of number needed to treat and under-treat with anti-DRP therapy and suggest that no single score tested improved both sensitivity and specificity compared to HCAP.

Application of a methicillin-resistant Staphylococcus aureus risk score for community-onset pneumonia patients and outcomes with initial treatment
-
Teshome BF, Lee GC, Reveles KR, et al. Application of a methicillin-resistant Staphylococcus aureus risk score for community-onset pneumonia patients and outcomes with initial treatment. BMC Infect Dis 2015;15:380. One of the first studies to evaluate clinical outcomes (30-day mortality) according to treatment thresholds for a DRP prediction score, in this case the Shorr MRSA score. This study showed that in patients with 0-1 points, adjusted mortality was actually increased with anti-MRSA antibiotics, with no difference for intermediate scores, and lower adjusted odds of mortality for MRSA-targeted therapy at threshold of at least 6.
