Since patients are often asymptomatic in an early stage and since ECG and echocardiograms are either normal or unspecific, myocardial inflammation often remains undetected. This cohort study provides evidence for the use of cardiac MRI to allow an early diagnosis of cardiac involvement in IIM patients which might also guide treatment decisions.

The study offers an explanation for dynamic muscle fatigue in systemic sclerosis. BOLD-MRI may be also used for the future quantification of vascular involvement in DM.
