This large CMR study carefully described the method used to assess potential IMH associated with MVO. A 'hypointense core' identified on T2-weighted imaging may indicate the presence of IMH, but may also represent MVO without IMH. A worse outcome in such patients nevertheless affirms the need to better understand the pathophysiology, and imaging, of IMH
