Marshall became known for his pioneering work in renal physiology and pharmacokinetics. He grew interested in therapeutic medicinal chemistry during World War I when he worked on mustard gas during his tenure in the Chemical Warfare Service in Washington, D.C. He received a Ph.D. in chemistry and an M.D. from the Johns Hopkins University and was chair of the new Department of Pharmacology and Pharmacokinetics from 1932 until 1955.

Sulfapyridine was the first drug to undergo FDA approval in the wake of the 1938 Federal Food, Drug, and Cosmetic Act requiring drug safety testing before sale and marketing, a law prompted in part by a disaster involving sulfanilamide. In 1937 the S. E. Massengill Company marketed "elixir sulfanilamide," a product containing the sweet-tasting but poisonous diethylene glycol, resulting in the deaths of more than one hundred people, including dozens of children who received the drug for a streptococcal infection.
