Rituximab was found to be as effective as cyclophosphamide for remission induction. This was a pivotal trial that formed the basis for FDA approval of rituximab for GPA and MPA in 2011.

This trial randomized 44 patients at a 3:1 ratio to receive intermittent cyclophosphamide or rituximab following 2 doses of intermittent cyclophosphamide. The trial did not meet its primary endpoint of superiority of rituximab, ?although rituximab appeared as effective as cyclophosphamide. A high mortality rate was seen likely reflecting the older age and renal insufficiency in the study population.

This study examined the experience with cyclophosphamide in GPA over four decades and described the evolution of use of this agent and the lessons learned regarding safety and efficacy over time.

Ann Intern Med. 2009;150:670-80. This study compared intermittent to daily cyclophosphamide. A similar rate of remission induction was seen although intermittent dosing appeared to be associated with a higher relapse rate. Daily cyclophosphamide had a higher frequency of leukopenia that occurred at times of decreased blood count monitoring. Intermittent cyclophosphamide provided a lower cumulative dose over time. This study suggested that both approaches are acceptable and provided guidance regarding blood count monitoring and cyclophosphamide duration.
