Nonalcoholic steatohepatitis: Summary of an AASLD single topic conference
-
Neuschwander-Tetri BA, Caldwell SH. Nonalcoholic steatohepatitis: summary of an AASLD single topic conference. Hepatology 2003; 37:1202-1219. A consensus document based on discussions held at an AASLD single topic conference on NASH. The document represents an important attempt to clarify definitions relating to the clinico-pathological spectrum of non-alcoholic fatty liver disease. A scheme for grading and staging NASH is proposed, based on a modified version of the system devised by Brunt et al. [16].

Hepatocyte apoptosis and fas expression are prominent features of human nonalcoholic steatohepatitis
-
Feldstein AE, Canbay A, Angulo P, Taniai M, Burgart LJ, Lindor KD, et al. Hepatocyte apoptosis and fas expression are prominent features of human nonalcoholic steatohepatitis. Gastroenterology 2003; 125:437-443. A recent study describing the importance of apoptosis as a mechanism of liver cell death in NASH. Markers of apoptosis and expression of the Fas receptor were significantly increased in NASH biopsies compared to normal controls and biopsies with simple steatosis, a finding which may have diagnostic and therapeutic implications.

Nonalcoholic steatohepatitis: Association of insulin resistance and mitochondrial abnormalities

Sanyal AJ, Campbell-Sargent C, Mirshahi F, Rizzo WB, Contos MJ, Sterling RK, et al. Nonalcoholic steatohepatitis: association of insulin resistance and mitochondrial abnormalities. Gastroenterology 2001; 120:1183-1192. Mitochondrial defects in the form of loss of cristae and paracrystalline inclusions were found to be present in patients with NASH, but not those with fatty change alone. This observation has implications for the histological diagnosis of NASH and for understanding mechanisms for hepatocellular injury.

Caldwell SH, Oelsner DH, Iezzoni JC, Hespenheide EE, Battle EH, Driscoll CJ. Cryptogenic cirrhosis: clinical characterization and risk factors for underlying disease. Hepatology 1999; 29:664-669. The lack of conspicuous histological features of NASH in cases where cirrhosis has supervened was also noted.

Nonalcoholic steatohepatitis: A proposal for grading and staging the histological lesions
-
Brunt EM, Janney CG, Di Bisceglie AM, Neuschwander-Tetri BA, Bacon BR. Nonalcoholic steatohepatitis: a proposal for grading and staging the histological lesions. Am J Gastroenterol 1999; 94:2467-2474. This represents the first attempt to devise a scheme for grading and staging the histological changes occurring in NASH. Although there are problems with applying certain components of the grading scheme, the Brunt system is now being widely used in clinico-pathological studies. It will no doubt form the basis for future studies investigating new approaches to the histological classification of fatty liver disease.

Interaction of iron, insulin resistance, and nonalcoholic steatohepatitis
-
Chitturi S, George J. Interaction of iron, insulin resistance, and nonalcoholic steatohepatitis [Review]. Curr Gastroenterol Rep 2003; 5:18-25. The role of iron overload as a co-factor for progressive liver injury in patients with NASH has been the subject of controversy. This article reviews previous studies investigating this matter and concludes that hepatic iron overload is unlikely to play an important role in the pathogenesis of liver fibrosis in patients with NASH.

Nonalcoholic fatty liver disease: A spectrum of clinical and pathological severity
-
Matteoni CA, Younossi ZM, Gramlich T, Boparai N, Liu YC, McCullough AJ. Nonalcoholic fatty liver disease: a spectrum of clinical and pathological severity. Gastroenterology 1999; 116:1413-1419. Another study investigating the natural history of NAFLD, confirming important differences in behaviour between simple steatosis and steatohepatitis. The importance of applying strict criteria for the histological diagnosis of steatohepatitis is also confirmed.
