Mitochondrial defects in the form of loss of cristae and paracrystalline inclusions were found to be present in patients with NASH, but not those with fatty change alone. This observation has implications for the histological diagnosis of NASH and for understanding mechanisms for hepatocellular injury.

Although there are problems with applying certain components of the grading scheme, the Brunt system is now being widely used in clinico-pathological studies. It will no doubt form the basis for future studies investigating new approaches to the histological classification of fatty liver disease.

The role of iron overload as a co-factor for progressive liver injury in patients with NASH has been the subject of controversy. This article reviews previous studies investigating this matter and concludes that hepatic iron overload is unlikely to play an important role in the pathogenesis of liver fibrosis in patients with NASH.
