"Sinus node (normal or pathologic) is governed by its own intrinsic properties and by extrinsic factors." The autonomic nervous system appears to be the predominant extrinsic factor. Few reports have emphasized the age-related changes of both extrinsic and intrinsic sinus node function in normal subjects as well as those with SND. The aim of this study was to define baseline normal extrinsic and intrinsic electrophysiologic sinus node function to determine changes with age and assess the role of the autonomic nervous system in those with and without SND or SSS.

In this retrospective 10-year analysis of 249 consecutive patients with pacemakers initially programmed to DDD mode, 62 (24.9%) had SSS and 180 (73.3%) atrioventricular block. Mean duration of follow-up was 32 months. At 60 months, 83.5% of those initially in DDD mode remained. AF was the most common reason for mode change. An atrial-based mode was maintained in 91% of patients and only 9% required shift to VVI.

Seventy-nine patients with SSS and chronotropic incompetence were randomly assigned to DDI or DDIR mode and followed up for 14 months, with a 6-month crossover. There was a significant decrease in palpitations and paroxysmal AF in both modes as compared with preimplant data.

Patients with atrial-based dual-chamber pacemakers were more likely to remain in sinus rhythm at 6 months (80%) than those with ventricular pacing (40%) or those without pacing therapy (55%).

The relationship between pacemaker type and mortality was analyzed in a random sample of Medicare beneficiaries (from 1988 to 1990) with minimum follow-up of 1 year. The use of dual-chamber (atrial-based) pacing appeared to be an independent predictor of survival at 1 year. The authors noted that the study had a number of flaws and reiterated the need for prospective clinical trials regarding mode selection.

In order to evaluate the relative importance of preserving normal ventricular activation sequence in permanent pacing, 11 patients with DDDR pacers for SSS but with normal conduction were studied. Comparison between AAI, DDD, and VVI pacing at rest and during exercise using invasive monitoring and radionuclide imaging was performed. AAI, with intact conduction, was associated with significantly increased cardiac output, decreased pulmonary capillary wedge pressure, and improved ejection fraction versus DDD with complete ventricular capture as well as VVI. This data indicates the importance of preserving normal ventricular activation whenever possible in permanent cardiac pacing.

PACE Pacing Clin Electrophysiol 1995, 18:11-17. There were 378 deaths in 2243 paced patients over a 5-year follow-up period. Sudden cardiac death accounted for 71 (18.7%), cerebrovascular accident for 56 (15%), congestive heart failure for 91 (24%), and (new) atrial fibrillation for 28 (8%). Sudden death was more often associated with patients paced for atrioventricular block, whereas the other three causes occurred more frequently in those with SSS.
