The unexpandable lung

In this interesting work the authors differentiate between trapped lung (caused by a benign fibrotic peel covering the visceral pleura and provoking mechanical problems) and 'lung entrapment', which would be secondary to active pleural inflammation, infection, or malignancy. In these cases, the underlying malignant or inflammatory condition is the primary clinical problem, and placement of a chronic indwelling catheter should be considered if the effusion is malignant. Pleural manometry was performed routinely during therapeutic thoracentesis and was useful for identification of different conditions leading to unexpandable lung

All patients were submitted to diagnostic thoracoscopy (using general anesthesia and double-lumen tracheal intubation), and then randomized to receive either talc poudrage at the end of the thoracoscopy procedure (group A) or bedside instillation of 20 ml of 10% povidone - iodine mixed with 30 ml of normal saline through the intercostal chest drain (group B). Two patients out of 22 in group A (9%) and 3 out of 20 (15%) in group B had a recurrence of the effusion requiring therapeutic measures during follow-up [mean: 22.6 (range: 8-48) months]. The authors conclude that povidone-iodine can be considered as a good alternative to thoracoscopic talc poudrage to ensure effective pleurodesis for patients with malignant pleural effusion due to metastatic breast cancer. Interleukin-8 activates coagulation and correlates with survival after talc pleurodesis

In this prospective study, IL-8 and tumor necrosis factor-alpha were measured in plasma samples from 231 patients with MPE before and after talc pleurodesis. Whole blood from 31 healthy volunteers was also incubated with IL-8, TNF-alpha and thromboplastin in vitro, and thrombin-antithrombin (TAT) levels were measured. The same stimulation of blood samples was repeated using doses of calibrated talc. Survival correlated inversely with IL-8 at 24 and 48 h in serum, and a significant correlation was also found between serum IL-8 and TAT levels. A positive dose-dependent correlation with TAT production was observed when blood was stimulated with IL-8 in vitro. It is concluded that IL-8 is involved in the activation of coagulation that may occur after talc pleurodesis, and might also be implicated in early death of patients with MPE

Use of indwelling pleural catheter in the outpatient management of recurrent malignant pleural effusion
-
In this multicenter study including 63 consecutive patients with MPE, a tunneled pleural catheter was inserted on an outpatient basis. Most of the patients (94.5%) reported an improvement in their respiratory symptoms (cough and dyspnea), and spontaneous pleurodesis was achieved following 34.9% of procedures. Empyema was observed in three cases, persistent chest pain in two, and tumor metastasis in three patients. (Article published both in Spanish and English)
