This large single institution series reported no positive surgical margins and a mean of 19 lymph nodes removed per case. Mean hospital stay was a relatively short 4.9 days. Though the follow-up interval is too small (21 months) to assess oncologic efficacy, the initial results demonstrate the feasibility of the robotic approach.

This well performed study of 120 patients undergoing a well defined extended PLND clearly demonstrated the increased prognostic efficacy of extended PLND. Lymph node metastases were present outside of the standard PLND field in 39% of patients, and the standard PLND template failed to identify 11% of lymph node positive patients.

J Clin Oncol 2009; 27:4314-4320. This report of 42 patients undergoing PET/CT prior to radical cystectomy/PLND utilized surgical biopsy to evaluate the accuracy of PET/CT in detecting lymph node metastases in patients with negative preoperative CT scans. Though the specificity of PET was high (94%), sensitivity was a disappointing 70%.
22. Swinnen G, Maes A, Pottel H, et al. FDG-PET/CT for the preoperative lymph node staging of invasive bladder cancer. Eur Urol 2009; 57:641-647. The results of this report on 51 patients, all of whom underwent radical cystectomy/ extended PLND following PET scans, were similar to the above, demonstrating high specificity but low sensitivity (46%) for detection of lymph node metastases. Interestingly, the authors also reported no significant difference in the diagnostic accuracy of PET when compared with CT alone.
