Low-energy femoral shaft fractures associated with alendronate use
-
A total of 70 people (59 women, 11 men), all low-energy subtrochanteric and midshaft femur fractures admitted to a trauma center in New York over 5 years, were reviewed. Of the 25 women on alendronate, 19 were identified as having a specific radiographic pattern (simple transverse fracture with a unicortical beak within a background of cortical hypertrophy). The mean duration of alendronate use was longer in those who had the fracture pattern compared to those without the fracture pattern

This study attempts to determine the risk factors for low-energy femoral fractures compared with a control group of patients with classic osteoporosis-related hip fractures of the femoral neck and intertrochanteric region. Bisphosphonate use was observed in 15 of the 41 subtrochanteric and shaft cases, compared with nine of the 82 hip fracture control subjects (OR, 4.44; 95% CI, 1.77-11.35). Patients with subtrochanteric/shaft fractures cases were more likely to be on long-term bisphosphonates than those with intertrochanteric and femoral neck fractures
