An electronic health record is defined by the Office of the National Coordinator for Health Information Technology as an "electronic record of health-related information on an individual that conforms to nationally recognized interoperability standards and that can be created, managed, and consulted by authorized clinicians and staff across more than one health care organization." An electronic health record also serves as the foundation for other functions such as clinical decision support. An electronic medical record is defined as an electronic health record that shares data only within its organization. These terms are often used interchangeably. While electronic health record best captures VistA, some of the IT adoption and capabilities frameworks are based on electronic medical record functionality and adoption; both have similar capabilities
