A multidisciplinary working group identified several high-priority areas in spasmodic dysphonia research, including improved assessment methods for differential diagnosis. A three-tiered approach was recommended, which included screening questions to suggest possible spasmodic dysphonia, a speech examination to identify probable spasmodic dysphonia, and nasolaryngoscopy for definitive diagnosis. The speech examination placed heavy emphasis on task-dependent sign expression as a means to distinguish spasmodic dysphonia from MTD. Promising pilot data are presented, although the authors acknowledge that a multicenter validation study is needed to examine the diagnostic performance of the algorithm more precisely

Sensitivity, specificity, positive predictive value, negative predictive value, positive likelihood ratio, and negative likelihood ratio were calculated to assess the diagnostic precision of acoustic analysis of phonatory breaks for correct identification of ADSD i.e. sensitivity vs. correct identification of MTD i.e. specificity. On the basis of the diagnostic indices reported, phonatory breakanalysis offers promise as a diagnostic test to distinguish ADSD from MTD, especially in men. However, one limitation of this type of acoustic analysis is the time-consuming nature of manually identifying and measuring phonatory breaks. Given the encouraging results, an automated process needs to be explored and validated to permit faster acoustic analysis of phonatory breaks of standardized speech samples
