Rheumatoid wrists treated with synovectomy of the extensor tendons and the wrist joint combined with a Darrach procedure

The authors presented a long-term follow-up study evaluating outcomes of extensor tendon and wrist synovectomy combined with a Darrach procedure. Patients were followed for an average of 11 years. The authors concluded that pain and forearm rotation had improved when compared to the opposite untreated side. However, carpal collapse and palmar carpal subluxation continued to progress equal to the opposite untreated side. The authors suggested that, in addition to the above-indicated treatments, stabilization of the wrist using radio-lunate arthrodesis in addition to wrist tendon transfer procedures to balance the wrist should be considered early. Flexor digitorum sublimis transfer for multiple extensor tendon ruptures in rheumatoid arthritis

This paper discussed the use of flexor digitorum sublimis transfer for multiple extensor tendon ruptures. In situations in which there is not enough donor tendons to reconstruct extensor tendon ruptures, the flexor digitorum sublimis can be useful. The integrity of this tendon for the long and ring fingers must be evaluated prior to tendon exploration. (Level IV evidence)
