This review article contrasts lung function tests with high-resolution chest CT imaging in CF patients. Chest HRCT imaging in CF children reveals structural lung changes such as bronchial wall thickness that occurs early in the disease process and also demonstrates that there is a large amount of nonhomogeneity of progressive lung pathology. In addition, structural changes seen on HRCT such as bronchiectasis represent irreversible progressive damage, and pulmonary function tests, which are a measure of global function, cannot discriminate localized or early lung damage in the CF child.

All HRCT scoring systems were reproducible and had good interreader agreement. All five scoring systems also showed significant correlations with pulmonary function measurements and with the bronchus-artery diameter ratio, a marker for bronchiectasis severity.

Pulmonary density measurements and percent air trapping better discriminated differences between groups than PFTs.

It is the first study of its kind to incorporate computer-generated quantitative CT outcome measures during a therapeutic intervention in CF subjects.
