The activity survey 2007 of the EBMT [6•] describes in details the Increase of transplant teams and numbers of autologous and allogeneic HSCT from 1990 to 2007 (Fig. 1) and illustrates the role of Gross National Income per Capita on the transplant rate in Europe (Fig. 2). It serves as a model to come for all stem cell therapies to come. This will be of increasing importance, as stem cell therapy has become a treatment vision for many patients with diseases that are not yet amenable to therapy. This might Include such a broad spectrum as from Parkinson's disease or diabetes to myocardial failure. Stem cell therapy is considered the treatment option of the 21 st century [7,8]. Time will tell, whether the promises will hold. There are as yrt no series on outcome published. Nevertheless, results on outcome of HSCT or any form of stem cell therapy will only be available years after initiation of such therapies. Other tools of data collection are therefore warranted. Information on trends presents such an option.

This report describes evolution in the Eastern Mediterranean Region (EMRO; as defined by the WHO; www.who.org), the current status and differences compared to Western European or United States of America HSCT teams. This novel analysis from a region with no large impact so far in the field of HSCT clearly showed the trends for the future. HSCT will become an important therapeutic tool for countries with restricted resources.

This report describes current modern approach with HSCT for thalassemia by integrating risk factors and novel supportive care strategies. These approaches are similar to those used in the treatment of childhood leukemia; risk of the disease and risk of the transplant define whether HSCT is used as primary or rescue therapy, or not at all. Early recognition and early assessment permits, today, to select children with thalassemia or sickle cell disease with high disease risk. These children are candidates for early HSCT. Transplant related mortality is very low, long-term cure a real possibility.

This recent summary shows that autologous HSCT can indeed halt progressive therapy refractory autoimmune disorders in a substantial proportion of patients. These data are the basis for several prospective randomized trials, comparing autologous HSCT with conventional nontransplant strategies. This Includes trials in systemic sclerosis, multiple sclerosis, Crohn's disease and systemic lupus erythematosus.
