Analysis of systemic sclerosis in twins reveals low concordance for disease and high concordance for the presence of antinuclear antibodies
-
Feghali-Bostwick C, Medsger TA Jr, Wright TM: Analysis of systemic sclerosis in twins reveals low concordance for disease and high concordance for the presence of antinuclear antibodies. Arthritis Rheum 2003, 48:1956-1963. This study examined concordance for SSc in monozygotic and dizygotic twins. Concordance for SSc was found to be similar in monozygotic and dizygotic twins, and overall concordance for SSc was low in the twins (4.7%). SSc-associated serum autoantibodies occurred exclusively in patients with SSc. The study showed that inheritance may play a role in the development of serum autoantibodies in the "healthy" twin sibling of a patient with SSc.

Sulli A, Ghio M, Bezante GP, et al.: Blunted coronary flow reserve in systemic sclerosis. Rheumatology (Oxford) 2004, 43:505-509. The authors of this study investigated whether the noninvasive determination of coronary flow reserve, as evaluated by transthoracic Doppler echocardiography, might be a potential method to detect early dysfunction of cerdiovascular system in 44 patients with SSc without clinical signs or symptoms of cardiac impairment. Coronary flow reserve was found to be lower in SSc patients, which may point to an indirect sign of heart involvement in SSc.

-
Derk CT, Sakkas LI, Rasheed M, et al.: Autoantibodies in patients with systemic sclerosis and cancer: a case-control study. J Rheumatol 2003, 30:1994-1996. In this case-control study, the authors found no statistically significant difference in the frequency of ACA and anti-Scl-70 antibodies among patients with SSc with and without the diagnosis of cancer. This study conflicts with previous studies that certain autoantibodies may represent risk factors for the development of cancer in patients with SSc

Correlation of serum anti-DNA topoisomerase I antibody levels with disease severity and activity in systemic sclerosis
-
Hu PQ, Fertig N, Medsger TA Jr, et al.: Correlation of serum anti-DNA topoisomerase I antibody levels with disease severity and activity in systemic sclerosis. Arthritis Rheum 2003, 48:1363-1373. This article revisits the utility of the anti-Scl-70 ELISA in the care of patients with SSc. Contrary to some previous studies, the authors found that serum levels of anti-Scl-70 antibody (measured by ELISA) correlate positively with disease severity and disease activity in SSc.

When combining the long PM-Scl-75 isoform in addition to the PM-Scl-100 in ELISA, the authors noted that there is an increase in the number of patients in whom anti-PM-Scl autoantibodies can be detected.

Autoantibodies to RNA-polymerases in Italian patients with systemic sclerosis
-
Bardoni A, Ossi P, Salvini R, et al.: Autoantibodies to RNA-polymerases in Italian patients with systemic sclerosis. Clin Exp Rheumatol 2003, 21:301-306. In this study of Italian patients with SSc, the authors found anti-RNA-polymerase antibodies to be less frequent in Italian patients than in Caucasian patients from the United Kingdom or the United States. Also, anti-RNA-polymerase I and III antibodies were associated with diffuse cutaneous involvement and male gender.

HLA class II alleles in systemic sclerosis patients with anti-RNA polymerase I/III antibody: Associations with subunit reactivities

Kuwana M, Pandey JP, Silver RM, et al.: HLA class II alleles in systemic sclerosis patients with anti-RNA polymerase I/III antibody: associations with subunit reactivities. J Rheumatol 2003, 30:2392-2397. Previous studies have failed to detect any statistically significant associations between the presence of anti-RNAP I/III antibodies and HLA-DRB1 or DOB1 alleles. The authors of this study found that the presence of anti-RNAP I/III antibodies was associated with HLA-DRB1*0405 and DQB1*0401 in Japanese SSc patients, and with HLA-DRB3*02 in Caucasians.

Am J Clin Pathol 2003, 120:596-606. There are limited data on antiendothelial cell antibodies in scleroderma. However, this study is consistent with other studies showing a possible relation with antiendothelial cell antibodies and pulmonary fibrosis.
