The authors undertook a number of psychophysical experiments in fibromyalgia patients and healthy controls on temporal summation of 'second pain' (TSSP), a phenomenon that is known to be increased in fibromyalgia patients and relevant for chronic pain development. The results provide strong support for alterations of central, not peripheral, pain sensitization for increased TSSP in fibromyalgia patients.

This is the first study investigating the effects of an experimental surgical incision in patients with fibromyalgia compared with healthy volunteers. Fibromyalgia patients reported significantly higher pain ratings than controls during and after incision. Increased pain ratings of patients compared with healthy controls paralleled atypical brain activation of patients in areas of the pain network. These results give further support to the hypothesis of changes in central mechanisms of pain processing in fibromyalgia. Furthermore, it supports clinical experience of higher pain ratings in fibromyalgia patients after surgery.
