In this randomized prospective trial, autofluorescence endoscopy significantly increased the diagnostic yield of surveillance endoscopy to detect intraepithelial neoplasia in the high-risk patient group with ulcerative colitis, even when compared to high resolution endoscopy in the control group. This illustrates a potential future use of AFI to screen large areas at risk.

This paper describes for the first time the use of molecular imaging of colorectal neoplasia by confocal endomicroscopy in humans. If this approach is combined with wide-angle macroscopic fluorescent imaging, a macroscopic red-flag technique for detection is combined with a microscopic technique for immediate in-vivo immunohistochemistry.
