The authors examine the outcome of EndoCinch plication in children with GERD (6-16 years). Patients demonstrated improvement in symptoms and QoL. The study was limited by a small sample size (17 patients). Significant improvement in heartburn and reflux symptoms persisted at 3 years. Four of the 17 patients required a repeat procedure. Evaluation of supplemental cautery during endoluminal gastroplication for the treatment of gastroesophageal reflux disease
-
This is the only clinical trial to examine the efficacy of supplementing sutures in endoscopic plication with heat cautery. In the small group (n = 18), patients randomized to cautery showed a trend toward improvement in heartburn and medication use; these trends were lost over the long term. Cautery did improve plication persistence, and studies in larger groups of patients may demonstrate efficacy

Gastroparesis associated with gastroesophageal reflux disease and corresponding reflux symptoms may be corrected by radiofrequency ablation of the cardia and esophagogastric junction
-
The authors demonstrated that RFA has the potential to correct gastroparesis in patients with concomitant reflux. At 6 months, 23 of 31 patients (74%) experienced normalization of gastric emptying. Four patients showed no improvement on their gastric emptying scan. There is no clear mechanism by which RFA corrects gastroparesis, although the authors postulate a reduction in transient LES relaxations

Gastroparesis associated with gastroesophageal reflux disease and corresponding reflux symptoms may be corrected by radiofrequency ablation of the cardia and esophagogastric junction. Surg Endosc 2008; 22:2440-2444. The authors demonstrated that RFA has the potential to correct gastroparesis in patients with concomitant reflux. At 6 months, 23 of 31 patients (74%) experienced normalization of gastric emptying. Four patients showed no improvement on their gastric emptying scan. There is no clear mechanism by which RFA corrects gastroparesis, although the authors postulate a reduction in transient LES relaxations.

The Stretta procedure versus proton pump inhibitors and laparoscopic Nissen fundoplication in the management of gastroesophageal reflux disease: A cost-effectiveness analysis
-
This Canadian study utilized a Markov model to determine the current 5-year costeffectiveness of laparoscopic Nissen fundoplication, the Stretta procedure and PPI therapy in the management of GERD. PPIs exhibited the lowest costs ($40 per symptom-free month) and the greatest efficacy in terms of symptom-free months, although not in quality-adjusted life years in which fundoplication was more effective but still more costly. The authors concluded that at the present cost, PPI therapy was most cost-effective
