Squamous cervical lesions in women with human immunodeficiency virus: Long-term follow-up
-
This study was aimed at estimating the frequency of and trends in abnormal Pap test results among women participating in the Women's Interagency HIV Study, in the United States. This is the first study to describe the evolution of cervical abnormalities in HIV-infected women during the last decade. It shows that, although the risk of cervical lesion falls over time, the proportion of HIV-infected women with abnormal Pap tests remains above 25%, with most lesions being lowgrade. In this population of women having proper access to screening and treatment of cancer precursors, progression of cervical lesions to cancer was unlikely during 10 years of follow-up

This study describes the prevalence, risk factors and prognosis of CIN among 153 HIV-infected women followed up between 1989 and 2006. Only one case of cancer of the uterine cervix was detected. Interestingly, in this study, the risk of CIN was not associated with decreased levels of CD4 cell count, duration of HIV infection, use of antiviral treatment or plasma HIV load, in contrast with most of prior studies. The conclusion of the authors is that regular follow-up by means of Pap smears is warranted in all HIV-infected women. (Pubitemid 351738841)

In only 32.8% of treatment samples, both margins were clear of CIN and a total of 55.6% patients had CIN at follow-up, despite margins being clear. Most failures and recurrences were of low grade, but one adenocarcinoma was diagnosed 4.2 years after therapy for CIN 1. This article describes efficacy of two excision procedures, loop excision of the transformation zone and electrosurgical conization, in obtaining complete excision of CIN in 80 HIV-infected patients. A high rate of 73% of complete excision was obtained by adapting the treatment modalities to the location of the lesion evidenced on colposcopy and to the shape of the cervix. This study highlights that appropriate surgical management leading to reduction in the rate of positive margins is needed for HIV-infected women. This may help in decreasing the risk of persistence or recurrence of lesions and thus, of cervical cancer

The objective of this prospective multicentre study was to develop an evidence base with which clinical guidelines regarding cervical surveillance and management in HIV-infected women could be developed. The data showed that cytology was sufficient for cervical surveillance, with HPV testing being less specific with poor predictive value, which was expected in this population with high rate of HPV infection.
