A comparison of 20118 patients with depressed LVEF (<40%) and 21 149 patients with preserved ejection fraction (>40%) found similar morbidity and mortality endpoints at 60 and 90 days. A risk and propensity-adjusted model found no significant relationships between discharge use of ACE-I/ARB or betablocker to 60 and 90-day endpoints among patients with preserved ejection fraction. Conclusions underscore the need for novel therapies and healthcare systems to effectively manage patients with heart failure and preserved ejection fraction

Serologic analysis of various components of the collagen turnover pathways were assessed in hypertensive patients and correlated with Doppler echocardiographic and clinical markers of DHF. Authors found a serum protein pattern consistent with active collagen turnover among patients with DHF. Levels of serum carboxyterminal telopeptide of procollagen type I, tissue inhibitor of matrix metalloproteinase (MMP)-1, amino-terminal propeptide of procollagen type III (all P<0.001), carboxy-terminal telopeptide of procollagen type I (P=0.008) and MMP-2 (P=0.03) were higher in more severe phases of diastolic dysfunction.
