Diamond burr polishing for recurrent corneal erosions: Results from a prospective randomized controlled trial
-
A double-masked, randomized study comparing diamond burr to epithelial debridement in 48 eyes with RCEs because of either traumatic or epithelial dystrophic changes. Twenty-five eyes underwent DBD and 23 eyes underwent epithelial debridement with noncontact 'sham' DBD. Results showed less major and minor recurrences, less need for repeated surgical interventions and less astigmatism in the diamond burr group compared to the epithelial debridement group. The authors feel DBD prevents RCE by both removing abnormal basement membrane to allow a smooth surface for re-epithelialization and by causing a reactive fibrosis to allow scarring and stronger epithelial adhesion. The authors feel DBD is superior to epithelial debridement alone due to a lower recurrence rate of RCEs, 15-18% in epithelial debridement alone versus 6% after DBD in this study
