In this large systematic survey of families of patients with terminal delirium (n = 560, with 70% response rate), Morita et al. report considerable emotional distress experienced by approximately 50% of families, and this distress correlated with the family experience of agitation, their perceived interpretation of delirium cause, and their perception about care they had received. This study has implications for care in the terminal setting in which majority of patients experience delirium, highlighting the need for improved communication between providers and families to prevent or alleviate family distress.
