Ultrasound Obstet Gynecol 2007; 30:687-696. This trial presents contradictory results and it is an important tool for understanding the bias among trials.
26. Fonseca EB, Celik E, Parra M, et al. Progesterone and the risk of preterm birth among women with a short cervix. N Engl J Med 2007; 357:462-469. This is the largest study in which nearly 25 000 pregnant women had a midtrimester cervical length assessment. Pregnant women presenting cervical length of 1.5mm or less were randomized for vaginal progesterone or placebo.

Vaginal progesterone is associated with a decrease in risk for early preterm birth and improved neonatal outcome in women with a short cervix: a secondary analysis from a randomized, doubleblind, placebo-controlled trial. Ultrasound Obstet Gynecol 2007; 30:697-705. This is a subanalysis of previous trial in which pregnant women with a short cervical length of 28mm or less present reduction in the PTB rate. This subanalysis reported effectiveness of progesterone in this population.

This is a very interesting study in which pregnant women presenting threatened PTB received intramuscular progesterone or expectant management. Their results are suggestive of effectiveness of progesterone in association with tocolytic therapy.
31. Borna S, Sahabi N. Progesterone for maintenance tocolytic therapy after threatened preterm labour: a randomised controlled trial. Aust N Z J Obstet Gynaecol 2008; 48:58-63. This study evaluated the progesterone maintenance after episode of preterm labor. Their results suggest that progesterone may be a therapy in this group.
