Steele JD, Christmas D, Eljamel MS, Matthews K. Anterior cingulotomy for major depression: clinical outcome and relationship to lesion characteristics. Biol Psychiatry 2008; 63:670-677. A recent study of anterior cingulotomy for major depression, which is noteworthy for its examination of lesions with MRI. It showed that such lesions affect both the cingulum bundle and the cingulated gyrus, and therefore not strictly a leukotomy. It also showed, but less convincingly, that the location of the lesions is indeed important for therapeutic response.

Schlaepfer TE, Cohen MX, Frick C, et al. Deep brain stimulation to reward circuitry alleviates anhedonia in refractory major depression. Neuropsychopharmacology 2008; 33:368-377. Not unlike psychosurgery, DBS is limited by the lack of certainty about the best targets for treatment. This study investigates the potential of nucleus accumbens stimulation in the treatment of depression in three patients with depression. Although there was an acute benefit, the long-term outcome of such stimulation is unknown. It is expected that investigations of this kind will eventually lead to a consensus on the best sites for DBS, and a matching of patient to lesion.
