Muller H, Handwerker G, Gebhardt U, et al. Melatonin treatment in obese patients with craniopharyngioma and increased sleepiness. Cancer Causes Control 2007; 17:583-589. This article describes the association of sleepiness, obesity, and salivary melatonin in a large case series of adult survivors of childhood craniopharyngiomas (79 patients) and hypothalamic pilocytic astrocytomas (19 patients) and controls. The patients with either tumor who were morbidly obese, BMI more than 4 SD, had lower salivary melatonin levels, and their EDS improved with nocturnal treatment of melatonin administration.

This study evaluated the preoperative MRI and the postoperative outcome of 66 children treated surgically for a craniopharyngioma and used the data to develop a preoperative classification system to grade hypothalamic involvement and thereby stratify the surgical treatment recommendations. Patients with hypothalamic involvement were treated with subtotal or partial resections instead of gross total resections, minimzing the surgical morbidity.

Lamont EW, James FO, Boivin DB, et al. From circadian clock gene expression to pathologies. Sleep Med 2007; 8:547-556. This article summarizes the current understanding of the human clock genes and their expression. Clock gene defects are responsible for the circadian rhythm disorders of advanced and delayed sleep phase syndromes, and are likely to be found to be important in many other diseases including cancers. This is a special issue of Sleep Medicine, which is devoted to various facets of circadian rhythm research.
