Clinically significant preformed RBC alloantibodies represent a special challenge in transfusion therapy. This remarkable study has extensively evaluated alloimmunization to RBC antigens in a cohort of 2000 transplant patients in Miami, mainly (95%) orthotopic liver transplantation recipients. Clinically significant pretransplant RBC alloantibodies were found in 5.8% of recipients. Strategies to provide successful transfusion support are discussed.

As in earlier studies along these lines, this new retrospective study of 433 adult cases between 1989 and 2004 indicates that RBC transfusions are an independent risk factor for survival after liver transplantation. The interesting point is the negative effect of platelet transfusion on survival shown by propensity adjusted analysis. Transfusion of platelets and RBCs is highly dominant in predicting 1-year patient survival.

This retrospective study of 209 cases aims to identify clinical variables independently associated with postoperative patient mortality. The number of RBCs and plasma units transfused is one of these variables. This study is also the first that shows that a history of anti-RBC alloantibodies negatively affects patient survival after liver transplantation. Boyd SD, Stenard F, Lee DK, et al. Alloimmunization to red blood cell antigens affects clinical outcomes in liver transplant patients. Liver Transpl 2007; 13:1654-1661. This retrospective study of 209 cases aims to identify clinical variables independently associated with postoperative patient mortality. The number of RBCs and plasma units transfused is one of these variables. This study is also the first that shows that a history of anti-RBC alloantibodies negatively affects patient survival after liver transplantation.
