Clinically significant preformed RBC alloantibodies represent a special challenge in transfusion therapy. This remarkable study has extensively evaluated alloimmunization to RBC antigens in a cohort of 2000 transplant patients in Miami, mainly (95%) orthotopic liver transplantation recipients. Clinically significant pretransplant RBC alloantibodies were found in 5.8% of recipients. Strategies to provide successful transfusion support are discussed.

As in earlier studies along these lines, this new retrospective study of 433 adult cases between 1989 and 2004 indicates that RBC transfusions are an independent risk factor for survival after liver transplantation. The interesting point is the negative effect of platelet transfusion on survival shown by propensity adjusted analysis. Transfusion of platelets and RBCs is highly dominant in predicting 1-year patient survival.

This retrospective study shows that patients with high MELD scores have more complicated intraoperative courses, with increased requirements for blood component transfusion and vasopressor use. The MELD score is positively correlated to transfusion of blood products. An interesting study showing the potential consequences of the MELD scoring system on blood transfusion demands.

This new multivariate analysis includes 460 cases at one center in Toronto between 1998 and 2004 and aims at predicting the risk of massive blood transfusion, defined as at least six RBC units in the first 24h. The strengths of the study are the sample size, the statistical model, and the use of preoperative variables only. In contrast to previous studies, the risk index that was developed seems to have a good discriminative performance.One of themain weak points is the questionable external validity of such a model when developed in one institution.

Liver Transpl 2006; 12:117-123. One of the most hotly disputed papers in this field. The authors compared a historical orthotopic liver transplantation series (1998-2002) with a recent one that was managed using an intraoperative blood conservation strategy: low central venous pressure through fluid restriction and eventually phlebotomy, blood salvage, low RBC transfusion trigger, no plasma. With this new approach, 79% of cases did not receive any bank blood. The new strategy apparently did not result in deleterious consequences on renal function or on other vital systems. These hypothesis-generating data call for confirmation, ideally by a randomized trial.

This retrospective study of 209 cases aims to identify clinical variables independently associated with postoperative patient mortality. The number of RBCs and plasma units transfused is one of these variables. This study is also the first that shows that a history of anti-RBC alloantibodies negatively affects patient survival after liver transplantation. In an interesting discussion section, the authors speculate that this association points to the deleterious consequences of so-called transfusion-related immunomodulation (see also the accompanying editorial [28]).
