Kojima H, Miyazaki H, Tanaka Y, et al.: Congenital middle ear cholesteatoma: experience in 48 cases. Nippon Jibiinkoka Gakkai Kaiho 2003, 106:856-865. This retrospective review of 48 cases of congenital cholesteatomas reported a Japanese experience with the symptoms and findings at initial evaluation, extent of disease, surgical procedures, and outcomes. The study results correlated with the usual finding of hearing loss; however, the authors found more posterior-superior middle ear space cholesteatomas, whereas in most Western literature reports the anterior superior quadrant is the most common site of lesions. The authors also reported one lesion that was found in the petrous apex.

The authors conducted a microscopic examination of fetal temporal bones, looking for signs of aspiration of squamous epithelium into the middle ear cleft via the Eustachian tube. None of the temporal bones examined showed any keratinizing epithelial cells or lanugo. This study did not support the theory of amniotic fluid and squamous epithelial aspiration as a cause of congenital cholesteatoma.

A retrospective chart review of patients with congenital cholesteatomas. The authors also studied the value of reoperative CT scans. Also a relations between the location and size of the cholesteatoma as well as the type of surgery, hearing levels, and rates of recidivism were reported. They emphasized the need for a high index of suspicion for, and early diagnosis of, this disease. Preoperative CT scans were highly recommended because microscopy is often insufficient to preoperatively determine the extent of the lesion and assess the ossicular chain. The more extensive preoperative findings correlated with a requirement for more extensive surgical procedures, poorer hearing results, and a higher rate of recidivism.
