The high prevalence of H. pylori among children in developing countries and the association with hypochlorhydria have suggested that this infection may predispose to acute or persistant diarrheal disease. In this prospective study conducted in 160 children aged less than 5 years from an orphanage in Thailand, the incidence of diarrhea was not significantly different between children by H. pylori serostatus.

Four cases of children presenting unexplained refractory iron deficiency are reported. Although the pathogenic mechanism is still unknown, reversion of anaemia after H. pylori eradication demonstrates a causal role of H. pylori in the pathogenesis of the anaemia.

Studies in adults suggested that eradication of H. pylori from the gastric mucosa is associated with long term healing of duodenal ulcer. Studies of duodenal ulcer disease in children are difficult to perform because of the low prevalence of the condition in this age group. Duodenal ulcer healed in 15 children after eradication of H. pylori, and all the children remained asymptomatic for a median of 37 months.

Ulcer healing was observed in 24 out of 25 children eradicated from Helicobacter pylori infection. During a mean follow-up of 2 years, the annual relapse rate was estimated to be 9%.
