This was the most recent, large randomized controlled trial comparing the efficacy and safety of LMWH with UFH for prophylaxis of VTE in patients with acute ischemic stroke.

These are the most recent guidelines from the Brain Trauma foundation (and others) about DVT prophylaxis in patients with TBI.

This is another review paper which is worth reading in case a stroke patient has confirmed VTE and should be treated with higher doses of anticoagulants.

55. Carlile MC, Yablon SA, Mysiw WJ, et al. Deep venous thrombosis management following traumatic brain injury: a practice survey of the traumatic brain injury model systems. J Head Trauma Rehabil 2006; 21:483-490. An inventory on prophylaxis and treatment of DVT in patients with TBI shows the diversity of strategies and thereby implicates the need for further systematic and prospective investigations.
