This is an important review of an analytic framework to recognize and distinguish between mediators and moderators so that most effective treatments and most likely responders can be identified.
2. Parker G, Roy K, Eyers K. Cognitive behaviour therapy for depression? Choose horses for courses. Am J Psychiatry 2003; 160:825-834. This is an excellent critique of the strengths and weaknesses of current empirical data on the role of cognitive therapy in the treatment of depression. It includes suggestions of key areas for future research.
3. Scott J, Sensky T. Methodological aspects of randomized controlled trials of psychotherapy in primary care. Psychological Medicine 2003; 33:191-196. This is a useful overview of the problems of undertaking RCTs of brief therapies.
4. Bolton P, Bass J, Neugebauer R, et al. Group interpersonal psychotherapy for depression in rural Uganda: a randomized controlled trial. JAMA 2003; 289:3117-3124. This is an important paper because it explores the use of brief therapies in different ethnic and cultural contexts.
5. Miranda J, Chung JY, Green BL, et al. Treating depression in predominantly low-income young minority women: a randomized controlled trial. JAMA 2003; 290:57-65. Women with MDD from impoverished backgrounds, where ethnic minority subgroups are over-represented responded well to ADM or CT and both treatments were superior to routine referral to community services. This RCT is important because it demonstrates that CT can be delivered to different socioeconomic, cultural and ethnic groups without any significant loss in efficacy.
6. Miranda J, Duan N, Sherbourne C, et al. Improving care for minorities: can quality improvement interventions improve care and outcomes for depressed minorities? Results of a randomized, controlled trial. Health Services Research 2003; 38:613-630. An important re-analysis of data from a large multi-centre RCT. It suggests that the observed reductions in depressive symptoms were primarily a result of the significant gains made by individuals from ethnic minority rather than Caucasian subgroups. The latter benefited much less from systematic care or CT, but interestingly still had better employment outcomes.
7. Araya R, Rojas G, Fritsch R, et al. Treating depression in primary care in low-income women in Santiago, Chile: a randomized controlled trial. Lancet 2003; 361:995-1000. This RCT that moved away from the traditional settings and populations usually involved in treatment trials of depression. The paper is additionally interesting as the therapy comprises a brief psycho-education package.

This has important implications for monitoring of progress and the development of all brief therapies.

This was an important study of the mechanisms of action of cognitive therapy that offers some support for the compensatory skills hypothesis.

35. Wampold BE, Minami T, Baskin TW, Callen Tierney S. A meta-(re)analysis of the effects of cognitive therapy versus 'other therapies' for depression. J Affect Disord 2002; 68:159-165. This carefully conducted meta-analysis controlled for therapist training, quality of therapies and other methodological problems. The authors demonstrate that when all these factors are taken into account all brief therapies have a similar level of effectiveness.
